I can say that the Draw-Tite # 75715 is a fit for your 2012 Nissan NV 2500 and I included an install we did on this exact vehicle. With that said, what I think is going on is the hitch is flexed a little. This does happen pretty often as the metal side brackets are not a super rigid steel.
